A valid means of identification
Your HMO card and enrolee number, if you have cover
Previous results, scans and discharge summaries
Every medication you are taking, including anything herbal
Your child's immunisation card for paediatric visits

The front desk calls you back to confirm the date and time, checks your cover if you hold an HMO plan, and tells you what to bring. If you do not hear from us within two hours during the day, call us directly rather than waiting.
